When it comes to excess gingival display, subtle enhancements can actually make a smile appear more natural than it did previously. Clinical Crown Lengthening (CCL) often does not need any restorative treatment after but can be paired with crowns or veneers, depending on functional and esthetic needs.

Extractions, Bone Regeneration & Implants

I enjoy the creative process of customizing treatment plans. This patient’s occlusion allowed us to fulfill the patient’s desire for conservative treatment. The pontic #24 was preserved as a cantilever and only #23 was extracted.

Large infections, such as #23 in this case, can cause a lot of bone loss. Loss of the buccal plate required grafting prior to implant placement.


Final restoration was completed four months after implant placement.

Free Gingival Graft (FGG)

My goal for all my patients is that they “need me as little as possible”--meaning we achieve successful, long-lasting results with minimal need for revisions. Free gingival graft (FGG) is one of my favorite options for accomplishing this. It remains the best option to increase the quality of soft tissue and eliminate an aberrant frenum pull. No man-made alternative has yet to outperform the predictability and longevity of the FGG.

FGG is frequently used for lower anterior recession where the tissue is too thin and the vestibule is too shallow for alternate grafting methods. I find the human body’s natural healing capability to be incredible. The graft takes within twelve days and the donor site on the palate completely regenerates itself within a month.


A common question I get is “how much will it hurt?” The answer is that it is sore for about a week about otherwise very manageable with NSAIDs.  The donor site is protected with cyanoacrylate for comfort while eating or talking.

CONNECTIVE TISSUE GRAFT

Connective Tissue Graft (CTG) is still the gold standard in treatment of recession. It is the most predictable option in gaining root coverage with gingival thickness. Root coverage is extremely important to protect the cementum from inevitable abrasion lesions.

I always recommend that patients be proactive about correcting recession because it will progress over time. Recession will never self-correct. Treatment is always less invasive and more successful before recession turns severe.

For this patient, a maxillary fixed hybrid on six implants was the appropriate choice. She had a long history of generalized severe chronic periodontitis severe bone loss, and frequent infections.


Maintenance of the teeth had an unfavorable prognosis and would not have resolved the patient’s esthetic concerns. She had developed a social anxiety as a result of the poor esthetics. 

We removed all maxillary teeth and placed six immediate implants with an implant-supported fixed provisional. Final restoration was completed four months later. Her son commented that she “looks more like herself.”

At my practice, I offer full arch implant treatments for patients seeking a long-term solution to failing dentition or complete edentulism. Full arch cases are among the most complex and rewarding treatments we provide. Each case must be tailored to the individual—there is no universal formula. The right treatment plan depends on the patient’s clinical presentation, their goals, and the long-term prognosis of their oral health.

A key part of the process is determining whether a patient is truly a candidate for full arch replacement. This therapy is not for everyone.

When full arch replacement is the better option, I work closely with the restorative doctor to customize every aspect of the case—from surgical planning to final prosthetic design.
